Standard Operative Procedures (SOPs)

  • The ‘Triage’ nurse should be vigilant and identify the serious/unstable children inthe OPD and expedite their care on priority basis. Such patients should be immediately transferred to the A&E department/ward accompanied by the doctor on duty
  • The stable patients will be initially seen in the medical filter clinics according totheir registration order. In complicated/problem cases the specialist on call will dothe assessment and decide the appropriate management
  • There must be proper documentation of patient’s profile in the OPD register.Write down the name, age, gender, hospital number and diagnosis. Mention anyreferral or admission
  • The patients should be instructed regarding the treatment (dose and duration) andneed for follow-up. Reassure them if no medicine is required
  • The prescriptions must be inlegible writing indicating patient’s name, hospitalnumber, date and strength of the drug. The drug should be written in capitalletters. It should be duly signed and stamped. All doctors should own a stampwith name and designation. In case of non-availability of stamp, the doctor must put his/her name in capital letters below his/her signatures
  • As the bed strength in the wards is limited, the patients with severe acute illnessesshall be given priority for admission over those with chronic disorders who mayneed admission just for diagnostic evaluation
  • Admission form must be filled in accurately indicating patient’s name, hospitalnumber, date & time of admission, name of admitting specialist, clinicalcondition, provisional diagnosis, and initial management. It should be properlysigned and stamped by the admitting doctor
  • After the initial assessment in the medical filter clinics, the patients may bereferred to the respective sub-specialties for ongoing care
  • When a patient needs consultation from another OPD clinic, he/she should beseen on priority basis with no more wait in line. The referring doctor shouldinform the concerned doctor on phone and/or send the patient accompanied by the peon/aya
  • The un-immunized/partially immunized children should be counseled and referredto the immunization department to complete the vaccine doses
